All students attending a PSM event (Girl’s Retreat, Beach Camp, etc.) must fill this out and turn it in.

PSM Personal Medication Form
Fill out and turn in only if you’re sending medicine with your student.

Scholarship Form
For up to a half scholarship towards student ministry events, like Camp!
